The Bringing It All Together grant is supporting a new partnership between South Bend Community Schools, Oaklawn, and the Paramount Health Data Project to better connect student health and academic success.
The initiative brings together data from nurses, counselors, and teachers to identify early warning signs and coordinate support before students fall behind. By aligning health care, mental health services, and education under one system, the partners hope to create a more proactive, whole-child approach that helps students thrive both in and out of the classroom.
